Providers are required to have on file an up-to-date certificate of immunization (SFN 16038) for each enrolled child in their program. All children must be up-to-date according to child care immunization requirements N.D.C.C. § 23-07-17.1 N.D.A.C. §§ 33-06-05 and 33-06-05-01 or have claimed exemption within 30 days of enrollment or be excluded from child care.
Early Childhood Licensing staff shall verify that providers have an up-to-date certificate of immunization (SFN 16038) in all child records they are reviewing at an announced licensing visit.
Early Childhood Licensing staff play a crucial role in supporting families to meet immunization and health and safety requirements. To assist families, staff will provide information on the ND Vaccines for Children (VFC) program.
